How to Become a Referring Agency

Agencies interested in becoming a referring agency with Bridging, Inc. must complete and return an Agency Application(PDF) .

Agency Criteria:
  • Agencies must have a Referring Agent assigned to each client.
  • A home visit must be conducted at the client's residence.
  • An agency must be a non-profit agency.
  • An agency must submit the Scholarship Application and current year budget if applying for scholarship.
  • An agency must maintain a no-show rate below 15%.  If an agency has a no-show rate above 15%, the agency must submit a plan to reduce their no-show rate.

Bridging Fee Structure:

Cost to Agency :
Appointments cost $45 per appointment.  During the first month of the year or the first time you purchase appointments, we offer a discounted rate of $40 per appointment.  This encourages agencies to purchase for the entire year and reduces administration.  Any number of appointments can be purchased.

Please note: cancellations require a 24-hour notice .  Without 24-notice of cancellation, your agency will still be charged $45 for the missed appointment. Any appointments that are not used in a calendar year are transferable to the next year.  Bridging's calendar year is January 1 to December 31.

 

Scholarship:
Bridging provides scholarship to agencies whose budgets are restricted.  Depending on the following criteria, agencies are eligible for a reduction in cost per appointment. Complete and return the Scholarship Application (PDF).

  • An agency must submit the current year budget and a completed Scholarship Application.
  • An agency must demonstrate financial need.  Examples include recent budget cuts and number of programs or service in proportion to budget size.
  • Scholarship funds need to be applied for every year.  Appication materials for the following year are sent in late October.

Agencies will receive notice of acceptance after we receive the materials requested and have had time to review the complete application.  Each Referring Agent who will be making referrals must attend Orientation before referrals can be made.

 

Delivery (Bloomington and Roseville site only):

Each delivery costs $125 and needs to be prepaid and arranged when the appointment is scheduled in advance.  Although $125 is below Bridging's actual cost, this fee will cover the entire move.

  • Clients have the option to move their own things or hire a mover other than Bridging.
  • The delivery will be the next day or within two days after the appointment.
